CHAPTER ONE
1.1 INTRODUCTION
Integration
of computerized documentation with knowledge based service in information
system is great in providing help for health professionals in their daily
practices, and also to improve their quality of service. Effort is continuously
been made in designing and developing reliable and economical data base system
to satisfy medical information system. This project is all about the design and
development of a database system for medical record of university college
hospital (UCH), and to shows how record and data can be handled and stored
within the system.
The
term medical record and health record are used somewhat interchangeably to
describe the systematic documentation of a single patient’s medical history and
care across time. The medical record includes a variety of notes or
documentation entered over time by
health professionals, recording observations and administration of drugs, test
results and reports. The maintenance of complete and accurate medical records
is a requirement of health care providers and is generally enforced as a
licensing or certification prerequisite.
The
proposed system will store patient’s registration record, card record,
patient’s medical record, ward been transferred to for treatment, and also
provide a means of storing unlimited information for a very long time as it possess
a very large capacity of storage. The record stored will be protected from
unwanted users, loss, misplacement and natural disaster. The system will process
data speedily and accurately provide information at when required.
The
database system will be used to store data, produce reports and handle
management enquiries in hospital. An information system is required to keep
track of day to day record to improve communication skills within the hospital.
Therefore,
it is hoped that the design and development of medical record database system
will provide a flexible and reliable management. This program is carefully
design to work in a stand-alone system or in a multi-user environment.
1.2 STATEMENT OF THE PROBLEM
It
is observed that university college hospital record operation is done manually
on staff and patients. The problems associated with the current method include
inaccurate data, poor quality of patient care and administrative inefficiency.
The tasks performed under this study are as follows:
i.
Careful
study of the manual operation of university college hospital medical centre that
brings delay in operation
ii.
Collection
of information on general problem confronting the medical centre.
iii.
Study
the identified problems, design and to develop a medical database system that
will help in solving these problems.
iv.
Develop
computer software that will help in the management of the medical record.

TIME SERIES ON ROAD ACCIDENT
CHAPTER ONE
1.0 INTRODUCTION
Worldwide over 1.2
million people die per annum as a result of road traffic accidents the 6th
cause of death, according to the World Health Organization [WHO 2004]. It is
also predicted by WHO that road traffic accidents will rise to become the 3rd
leading cause of death and Injury by the year 2020. The United Nations UN
declared this an issue of great concern.
A road traffic accident is when a
road vehicle collides with another vehicle, pedestrian, animal or obstacles.
The accidents can result in injury, property damage and even death. In this
project work, a road traffic accident is defined as accident which takes place
on the road between two or more object; one must be any kind of moving vehicle.
The morbidity and mortality burden in
the country is in a rise due to frequent occurrence of road traffic accidents.
This is due to a combination of factors such as poor road, poor traffic
infrastructures, drunkeness, excessive speeding, failure to obey traffic rules
and regulations e.t.c.
Concerns about the
rising incidence of
fatal road accidents has compelled stake holders to seeking means to
curb road
accident. That is why a body was formed and incorporated to monitor road
traffic accidents for safety, this body is known as the Federal Road
Safety Commission [FRSC]. Following the UN declaration in 2011 that the
period 2011 –
2020 be adopted as the UN decade of action for road safety, the Federal
Road
Safety Commission [FRSC] in Nigeria set out to adopt and domesticate
action
plans by developing a number of programmes suitable for every road user
in the
country. Despite this effort, the problem still persists.
This project work makes use of Time
Series analysis to determine frequency and rate of road accident with special reference to Ibadan and environs.
1.1 BACKGROUND OF THE STUDY
In this project, we analyses road accident and death occurrence based on data collected from the
FRSC [Federal Road Safety Commission] Ibadan, Oyo State for the period of 10
years.
Ibadan is a heavily
motorized city with a large network of road and transport system. It has a high
rate of road transport accident and the tendency is on this rise.
